We understand how nonmedical factors affect patient outcomes. These social determinants of health (SDOH) are highly relevant to understanding and helping patients navigate their treatment journey. It is imperative to plan for and design patient service programs to help patients overcome these hurdles.
In our white paper Understanding the Importance of Utilizing Social Determinants of Health, VP, Patient Access & Strategic Engagement, Dana Edwards, details the areas in your patient services program that can be effectively tailored to address SDOH. Now, you can work through this scorecard to determine where your program is and is not meeting the SDOH needs of your patients.
